Group Day Habilitation Program
If you are looking for a day habilitation program that offers a variety of uniquely enriching experiences, Triform Camphill Community may be the place for you. Certified by the New York State Office for People with Developmental Disabilities, this 8:30am – 1:00pm weekday program is designed for young adults with intellectual and developmental disabilities who reside locally in their own homes, family care homes, and other local settings. Transportation is provided to all day program participants who reside within a 15 mile radius.
Appropriate for young adults, aged 18 and over at the time of admission, who are seeking a day habilitation program or who are “aging out” from their high school.
The curriculum focuses upon the young adult as they emerge from the teenage years into young adulthood. Reflecting this emergence, the curriculum provides experiences in vocational work, physical movement, cultural arts, life skills, crafts as well as supporting the need for appropriate social skills, artistic development and self-knowledge. Triform attempts to accompany and assist each person during this unique time in their lives.
An average day consists of land-based activities, practicing life skills and working in craft studios.
Funding for Triform’s Day Habilitation Program is available through the New York Home and Community Based Services (HCBS) Waiver, local school districts and private sources.
Day Habilitation Program Closings Procedure:
In the case of inclement weather, we follow the closing, delay, or early dismissal decisions of the nearby Taconic Hills School District.
