Group Day Habilitation Program

If you are looking for a day habilitation program that offers a variety of uniquely enriching experiences, Triform Camphill Community may be the place for you. Certified by the New York State Office for People with Developmental Disabilities, this 8:30 AM – 3:45 PM weekday program is designed for young adults with special needs who reside locally in their own homes, family care homes, and other local settings. Transportation is provided to all day program participants who reside within a 15 mile radius.

Appropriate for young adults age 18-25, who are seeking a day habilitation program or “aging-out transitional program” from their high school, Triform’s Day Habilitation Program offers the same high quality curriculum offered in its Residential Program, including after hours and weekend community events.

The curriculum focuses upon the young adult as he or she emerges from the teenage years into full adulthood. Reflecting this emergence, the curriculum provides experiences in vocational work, basic academics, physical movement, cultural arts, home life skills, crafts, as well as supporting the need for appropriate social skills, artistic development and self knowledge. Triform attempts to accompany and assist each person during this unique time in their lives, ideally providing the support, education and experience they will need as an adult.

An average day consists of work in one of eight vocational areas in the community (weavery, farm, garden, bakery, estate, forestry, home economics and pottery), joining one of the households at lunchtime for a home-cooked meal, and participation in various classes, therapies or excursions in the afternoon. As each young person progresses, they will be accompanied by our job coach while they engage in volunteer work in a local business or other work setting in Columbia County.

An individual can begin the program at any point during the year. Funding for Triform’s Group Day Habilitation portion of the program is available through New York Home and Community Based Services (HCBS) Waiver, local school districts, and private sources.
