Student Admission

Who May Apply
First-time applicants, applying to Triform, must be between 18 and 25 years of age. Triform will consider individuals with mild or moderate developmental disabilities or mental illness, learning disabilities, social or emotional difficulties, or temporary or long-term adjustment problems.

First Step
An initial telephone conversation, with the admissions director, will establish whether or not a visit and interview is appropriate. If you wish to make an inquire, please call the director of admissions, Tim Paholak at 518-851-9320, e-mail him at admissions@triform.org, or fill out our Contact Request Form.

Initial Visit and Interview
The applicant visits Triform, accompanied by parent, guardian, or advocate, and meets with members of the community. Following the visit, the applicant (or parent/advocate) may write or call to request a trial visit. The Admissions group will then consider if the applicant is eligible for a trial visit.

“Over the years, I have had the
opportunity to get to know
many of the co-workers. I’ve seen
their dedication first hand and
the outstanding results that
patience and understanding
can bring to the lives of those
who need care and hope the most.”
- Anne, Jamie’s mother.

Trial Visit
A trial visit of approximately four weeks is required prior to an admission. During this visit, the perspective student experiences every aspect of community life. Near the end of the trial, the Admissions Group will meet with them to talk about the experience and consider the next step. If Triform decides to accept the applicant, and the applicant wants to join Triform, he or she usually returns home to think over and discuss their decision with their family. If a decision to commit to at least a one year stay is made, the new resident joins the community as soon as space is available.

Duration of Stay
The student program lasts a minimum of three years, followed by the apprenticeship phase which, depending on the individual, lasts three to five years, A person may live in Triform until the end of the apprenticeship, as long as they, their parents or advocates, and the community feel that Triform is the right place for them. Triform asks that the parent/guardian be responsible for the transition out of Triform after their graduation from the apprenticeship program. Triform will be as supportive as possible during this process.
